Hello All,
The server for this site will not allow me to serve up any scripts that collect data submitted by online forms. So, I'm creating a work-around. Unfortunately, it may require an added step for you. If you have Adobe Acrobat Reader 7 or Adobe Acrobat 7 then you're set to go. If not, don't worry. You can download Acrobat Reader 7 for free.
System Requirements for Acrobat Reader 7:
Microsoft® Windows 2000 with Service Pack 2, Windows XP Professional or Home Edition, or Windows XP Tablet PC Edition,
Mac OS X v.10.2.8 or 10.3
Click here To download Adobe Acrobat Reader 7.
Once you have installed Reader, Click on the document below. It should open in your browser window. Fill out the form and click send. This will open your default email application with a new message addressed to me and containing the necessary attachment . Feel free to add any comments or just click send. You should also be given the option to print the form for your own records.
